The March 12, 2015 death of a nursing home resident (Juanita Simmons) at the Midwest Rehabilitation and Respiratory Center in Belleville has prompted an investigation.

The St. Clair County Coroner’s Office has an “open and active” investigation into the death of Simmons, who was found dead Thursday morning at Midwest Rehabilitation and Respiratory Center, 727 N. 17th St. in Belleville. Nursing home administrator Rebecca Garcia could not be reached, and previously declined comment.
The article discusses alleged mistreatment and potential nursing home abuse of Simmons, and also highlights 20 complaints filed with the Illinois Department of Public Health against Midwest in the past year.
